The Photograph From Ten Years Ago

A longevity physician who cannot wash his face at night, a twin study, and what compounds on your face across a decade if you start tonight.

Peter Attia has built a career on the science of living longer and better. He wrote the bestselling book on it. He runs a medical practice devoted to it. He is, by any reasonable measure, one of the more informed people alive on the subject of how bodies decline.

He also cannot make himself wash his face before bed.

He confessed this on his own podcast in 2025, seated between two aesthetic medicine specialists. Before recording, he had looked at a photograph of himself from ten years earlier, and the difference had genuinely startled him. Which led to the thought that follows anyone who has done the same thing: if the last decade managed that while he ignored the entire subject, the next one will manage more.

His explanation was disarmingly honest. He avoids his appearance because he does not understand it, so he attends to the things he does.

That is not laziness. It is a very particular problem, and most of us have it.

The trouble with a ten-year signal

Skin is the only thing in your life that gives you feedback on a decade's delay.

Exercise reports back in weeks. Sleep reports back tomorrow morning. Skin says nothing for years and then hands you the entire bill at once, usually while you are scrolling past an old photograph on a Sunday afternoon.

This is precisely the structure human decision-making handles worst. There is no signal to respond to. The cost of skipping tonight is nothing. It is nothing every night for roughly three thousand six hundred and fifty nights, and then it is not.

Which is why the useful question is never what went wrong in the last decade. It is what begins compounding tonight.

What the twins settled

In 2009, Dr. Bahman Guyuron published a study in Plastic and Reconstructive Surgery conducted somewhere improbable: the annual Twins Days Festival in Twinsburg, Ohio. His team photographed identical twin pairs and asked an independent panel to judge which sibling looked older.

Among twins who differed in smoking history, a gap of five years or more produced visibly different faces, concentrated in the middle and lower thirds. A companion analysis of 186 pairs examined sun exposure.

Identical genes. Different lives. Different faces.

You have almost certainly seen a photograph from this research without knowing its origin. Two sisters, side by side, captioned as one who smoked and tanned for sixteen years against one who did not. Snopes examined it in 2025 and again in 2026. The photographs are genuine and they do come from Guyuron's work. The caption is wrong. The images illustrated sun exposure rather than smoking, and no sixteen-year period was ever described.

The correction is more useful than the myth, because it reveals what the caption conceals. The difference between two genetically identical faces was produced by ordinary accumulated exposure, not by one dramatic vice.

Four processes, running simultaneously

The fat pads shrink. A young face carries around eleven fat compartments on each side, plump and evenly distributed, cushioning the space between skin, muscle and bone. By the late sixties most have visibly diminished. This is the change that makes women say they look tired without being able to name why, and it runs backwards from the rest of the body, where fat tends to arrive rather than depart.

The bone recedes. The facial skeleton loses volume, particularly around the eye sockets and the jaw, so the scaffolding beneath quietly withdraws.

Collagen declines, and for women there is a cliff. Ordinary ageing takes roughly one to one and a half percent a year from early adulthood. Menopause stacks something far steeper on top: figures from the British Menopause Society put it at around thirty percent of skin collagen in the first five years, then about two percent annually for the two decades after.

And the surface accumulates damage, driven overwhelmingly by sun.

A ten-year plan is a plan against four separate mechanisms. Which is why no single product has ever solved it, and why every product that claims to is selling you something else.

What compounds

Sun, including the kind you never register: through the car window, at a desk, on grey days. It never feels like exposure, which is exactly the problem.

A retinoid, which carries the best evidence in skincare and the evidence is structural rather than cosmetic. In a 1993 study published in the New England Journal of Medicine, biopsies from twenty-six people compared sun-exposed forearm skin against skin from the buttock, which had been protected all their lives. The exposed skin was producing fifty-six percent less collagen. In the treatment arm, ten to twelve months of tretinoin produced an eighty percent increase in collagen formation against a fourteen percent decrease on the vehicle cream alone.

Barrier care, the least glamorous item on any list and the one that decides whether you can tolerate the others.

And stress, which is more measurable than it sounds. A 2024 study in the Journal of Cosmetic Dermatology found moderately stressed subjects showed around thirty-three percent more severe texture change and fine lines than mildly stressed ones, though it is worth knowing the authors worked for a cosmetics company.

None of these does anything dramatic in a week. All of them do something decisive in a decade.

The plan is smaller than you think

The instinct, confronted with a ten-year problem, is to build a ten-year solution. Something serious. A regimen.

That instinct is precisely why most women who resolve to take their skin seriously have quietly stopped by March. A routine you abandon contributes nothing across a decade. A boring one you keep contributes everything.

The version that survives contact with real life is roughly this: protection every morning including the mornings it feels absurd, a retinoid at whatever frequency you can tolerate without quitting, something that supports your barrier, and enough repetition that none of it remains a decision.

Begin the retinoid twice weekly rather than nightly. Irritation is the single most common reason women abandon the best thing available to them.

And if you are reading this at fifty rather than thirty, the framing holds; it simply applies to the decade ahead. Those retinoid studies were run on skin that had already taken the damage, in people in their forties, fifties and sixties. Improvement was available to them. What changes with a late start is the ratio of effort to result: prevention is nearly free, correction is expensive and gives back less.

The part nobody measures

Here is what makes this genuinely difficult rather than merely inconvenient. You cannot watch it happen.

Change at this pace is invisible in a mirror. You look at your face every day, which is exactly why you cannot detect a trend in it. Attia noticed only because he stumbled across a photograph, and by then the decade was spent.

That is a measurement problem, not a failure of character. And measurement problems have solutions. Photograph your face at intervals and you have a record. A record gives you a trend. A trend gives you feedback on a timescale you can respond to, rather than waiting ten years for the verdict.

Your ten-year self does not require anything heroic of you tonight. She requires only that you start keeping score, and then do the dull things often enough that the score is worth reading.
